1. Who may use the service
JV Clinical Technologies tools, including Appeal Builder, are for healthcare professionals, practices, billing specialists and administrators who are authorized to use them for their work, such as preparing appeals. You must be at least 18 and able to enter a binding agreement.
2. De-identified information only
You agree to submit only information that has been properly de-identified before submission, and not to submit names, dates of birth, member or authorization numbers, record numbers, contact information, addresses, patient-related dates, or any other information that could identify a patient. You are responsible for preparing and verifying documents. Automated screening in the service does not determine whether information is de-identified. If you submit identifying information, you agree to notify us promptly.
3. Drafting support, not professional advice
The service provides clinical evidence analysis and draft appeal letters. It does not provide legal advice, medical advice, coverage determinations or representation, does not submit appeals, and does not guarantee approval, reimbursement or any outcome. All output is a draft that must be reviewed and approved by an appropriately authorized professional, who remains responsible for its accuracy and use. You must verify applicable payer, plan, state and federal appeal requirements and deadlines.
4. Accounts and security
Keep your sign-in credentials confidential. Two-factor authentication is strongly recommended. You are responsible for activity under your account.
5. Plans, fees and cancellation
Each tool is offered on monthly subscription plans shown on the Pricing page. For Appeal Builder, a letter counts toward your plan's monthly allowance (Starter: 10 letters; Professional: 25 letters; Practice: 50 letters) only when it is successfully generated. Allowances reset each billing period and do not roll over. A practice owner or admin manages plans and billing for the practice. You may cancel at any time; cancellation takes effect at the end of the current period. [OWNER: confirm refund policy.]
Free trial and automatic renewal. Each practice may receive one free trial of 14 days including 2 letters. A payment card is required to start a trial. Unless you cancel before the trial ends, your subscription starts automatically and the plan price is charged to your card on the day the trial ends and every month after that until you cancel. You can cancel anytime from Account, Manage billing. We send a reminder before the trial ends. A trial is limited to one per practice and per payment card; if a card or practice has already received a trial, billing starts immediately. [OWNER: have counsel confirm automatic-renewal disclosures for the states where you sell.]
Pay per letter. You may also buy Appeal Builder letters without a subscription for $4.50 each. Purchased letters do not expire, are used after any monthly plan allowance, and are not refundable once used.
6. Acceptable use
Do not attempt to access other users' accounts or data, interfere with the service, upload malicious files, reverse engineer the service, or use it for any unlawful purpose.
7. Disclaimers and limitation of liability
[OWNER: insert disclaimer of warranties and limitation of liability language prepared by counsel.]
8. Changes and governing law
[OWNER: insert change-notice process, governing law, venue and dispute resolution terms prepared by counsel.]
